Claim Procedure
Submit The Following Documents:
- Certified and clear ID copy of the deceased member.
- Certified and clear ID copy of the main member / policy holder or claimant.
- Certified and clear death certificate (serial numbers must be visible).
- Insurer claim form.
- Copy of the BI1663 (page 1-3).
- If the deceased died of unnatural causes, a completed police report.
- Authority /Mandate to pay the undertaker, where the parlour must be paid.
